- NICOP will be issued by the NADRA.

- Fee payment mode is cash only and will be collected at NADRA counters.

- CNIC of the applicant (if available)

- Witness from Blood relative (In person) / Attested affidavit from Gazetted officer.
- CNIC of the Applicant (Expired)
- Document required accordingly to prove the correction request. (In case renewal + correction)
- For marital status (Nikah Nama) and witness of husband. (In case renewal + change in marital status)
- Valid Passport
- Document required as per the nature of Modification / correction
- For marital status (Nikah Nama) and witness of husband (In Person) and witness of father / mother / brother (In person) if applying of the first time.
